Most toilets in El Salvador are used with toilet paper placed in the bin rather than flushed, especially in older buildings and smaller towns where plumbing can clog easily. In hotels, malls, and newer homes you may be able to flush paper, but if there is a bin or a sign asking you not to flush, follow it and carry small bills or coins for paid public restrooms.
